Staff now have access to a broader range of health and care specialists and the ability to provide better continuity of care, leading to improved staff satisfaction.


By working more closely together our teams of staff are able to discuss patient needs and develop shared care plans to make sure they get the right care, at the right time, in the right place.


Staff will build on their established relationships across the borough so they can suggest the very best path for the individual or family taking in to consideration the individual's opinion and that of the support services around them.


They will be able to build on skills and talents within the existing workforce and provide new opportunities and development. They will take a proactive approach rather than responding to crises and they will be supported by their partner organisation to make decisions to find new and better ways of working together.
